1、 Firstly, organize safety training for the attendees, which includes watching the warning educational video "8 • 6 Major Coal and Gas Outburst Accident at Zimujia Coal Mine in Panzhou City, Liupanshui City, Guizhou Province". This warning educational film provides a detailed analysis and explanation of the causes of the accident, the main problems exposed, the nature of the accident, and the lessons learned.

2、 Each unit reported on the work of safety production and safety production standardization management system in October, as well as the shortcomings in the work, and raised the problems that need to be solved to the company. They also planned and arranged the work for November.

3、 The head of the company's safety supervision department, Qu Jiabi, reported on the company's safety production inspection, safety hazard investigation, various coal mine injuries and their causes, and hidden danger inspection and punishment in October, and arranged the safety work priorities for November. Other production departments of the company have made special arrangements for the work in November.

3、 Chief Engineer Zhu Zeng proposed the following requirements for the work in November: 1. Revise the geological report of the mine every 8-10 years according to the requirements, and complete this work before December. The company's geological survey department should supervise and track it well. 2. Water prevention and control work, no matter how urgent the project is, must be carried out in advance, while considering drainage capacity. The key points for water prevention and control in various mines in November: 11203 transportation roadway and 20302 return airway in Zhongheng Coal Mine; Honghui Coal Mine 20301 Upper Return Air and 11703 Return Air Lane Encounter Old Kiln; Exploration and drainage of transportation lanes 11123, 11212, and 11212 return air lanes in Yangchangpo; Mangyuan coal mine fault fissure water. 3. Strengthen the collection and management of on-site basic data. The data of the 11124 mining face on Yangchangpo should be collected well to have a clear understanding. The locations of the drilling site, water pits, and faults should all be collected as shown in the above diagram. These are all hidden disaster causing factors and should not be taken lightly. 4. In terms of ventilation system, each mine should carefully sort out the existing problems, including the supplementary works that need to be supplemented, the unnecessary tunnels that need to be removed, and the ones that need to be removed to reduce unnecessary air consumption. The construction of Zhongheng Coal Mine's 1500 and 1570 specialized sections should be expedited. Construction of 12304 return air, pay attention to the relationship with the wellbore. Mangyuan Coal Mine has organized the on-site construction well. Currently, it is a single fan with hidden dangers, and emergency response work needs to be carried out well. Strengthen the investigation of ventilation system diagrams, and the ventilation and defense department should conduct inspections. Strengthen the management of communication and defense facilities, and do not use temporary ventilation ducts or steel mesh for obstruction. 5. In terms of gas management, on-site management work has been strengthened. The first half of the year was relatively stable, but there were multiple instances of gas exceeding the limit in the second half of the year. 6. Maintain the monitoring system by contacting the manufacturer. 7. In terms of drainage management, we strictly follow the design and construction to achieve the expected results, while preventing situations such as pipe detachment, pipe cracking, and delayed drainage due to tunnel deformation, which may affect the drainage effect. 8. According to the requirements of the Notice on Holding the "Panjiang Cup" Coal Mine Youth Engineering and Technical Personnel Safety Regulations and Measures Competition in Liupanshui City (Liupanshui Qinglian Fa [2021] No. 4) and relevant work tips from the Liupanshui Energy Bureau, timely personnel from each mine are organized to participate in the "Panjiang Cup" Coal Mine Youth Engineering and Technical Personnel Safety Regulations and Measures Competition in Liupanshui City. Before November 8th, the safety technical measures of each mine participating in the competition are first reported to various departments of the company. After initial review by each department of the company, they are promptly modified and then reported to the participants. 9. The mining continuity plan for 2022 should be reported to the company by each mine before November 10th. 10. Technical demonstration of blasting mining face, each mine shall first make technical demonstration and submit it to the company for approval. 11. The design of the 1200 water tank pump room in Zhongheng Coal Mine needs to be optimized, and the optimization plan should be submitted to the company; The Yangchangpo well area should make good use of the drilling of the 11123 transportation roadway fault, explore the fault situation clearly, and lay a foundation for the next step of work. The plan should be submitted to the company within 10 days. Yangchangpojing District should do a good job in applying for second level quality standardization, and contact and learn more from Zhongheng Coal Mine to avoid detours; Mangyuan Coal Mine should prepare for the joint trial operation and prepare various materials.

4、 Vice General Manager of Production Shi Minghua reported on the completion of production plans for each coal mine in October and made plans for safety production in November. He put forward the following requirements: 1. Emergency broadcasting systems for each mine must be reported as soon as possible, and all must be completed by the end of November. The supply and marketing department should track the arrival of equipment. 2. The mining continuity plan shall be reported according to the time nodes. The salary distribution plan for each mine in 2022 shall be submitted to the company before November 20th after soliciting the opinions of the resident managers. The assessment method of the company will be carried out simultaneously by each department and discussed centrally by the company. 3. Work of each mine: (1) Installation and improvement of the main fan, gas drainage pump, and drainage system in Mangyuan Coal Mine. b. Prepare for the installation of comprehensive mining equipment and the installation of comprehensive mining lines, and organize the comprehensive mining installation personnel. c. Improve the relevant engineering, data, and reports for joint trial operation, and promptly seek third-party implementation of related work. d. Improve the second power supply line; (2) Preparation of the A.10708 mining face transportation system and maintenance of the 16 machine rail stone gate at Honghui Coal Mine. b. Organize the construction of the second mining area well. c. Carefully organize and ensure the continuity of mining operations. d. Implement the relevant preparations for extending the certificate. e. Strengthen coal quality management, and after the system is perfected, focus on the separation and transportation of coal gangue; (3) Yangchangpo Well Area a. Strengthen construction quality management engineering, come up with acceptance management methods, and do a good job in quality acceptance work. b. Strengthen safety education and training for new employees, implement and track safety management systems, and reduce work injury rates. c. The renovation of the return air on 1124 should be carried out in accordance with the requirements of relevant documents. d. Regarding the issue of small mining face recycling, prepare the conditions for mining face recycling and consider the work from multiple perspectives and angles; (4) Zhongheng Coal Mine a. The management ideas of the mining team should be unified and work together. After reaching a consensus, communicate and implement various tasks among all levels of management personnel in the mine to prevent fragmentation. b. Strengthen the safety management of the initial mining of 12001 mining face and the final mining of 115-101 mining face, and ensure that safety facilities and protection are in place. 115-101 mining face must have the conditions for recycling before November 20th. c. Accelerate the preparation of the 11204 return air bottom pulling and comprehensive excavation machine, and have the conditions for backfilling. d. To increase the maintenance and repair work of the system, it is necessary to ensure the stability and reliability of the return air system and production system. e. Intensify safety supervision and reduce work-related injuries. f. Carefully organize the construction of 12304 return air excavation and strive to complete it by the end of December. 4. The supply and marketing department tracks the arrival of production materials to ensure production.

5、 Zhou Xianbing, Deputy General Manager of Mechanical and Electrical Engineering, put forward the following requirements: 1. Each mine's mechanical and electrical equipment and materials should be planned, and the company should ensure timely supply according to the importance and urgency. 2. This month, conduct a survey on the eliminated equipment and eliminate it in a timely manner. 3. Strengthen the management of heating equipment, check the aging condition of the circuit, and ensure that the switch is turned off promptly when people leave. 4. Install the gas pump of Mangyuan Coal Mine properly. 5. Zhongheng Coal Mine and Yangchangpo Well area should do a good job in gas power generation. 6. Prepare the route and materials for Honghui Coal Mine.

6、 Chen Hua, the director of the management office, has put forward requirements for the construction of the second mining area of Honghui Coal Mine.

7、 Peng Xianglai, the Chief Financial Officer and Minister of Personnel, pointed out that: 1. The company's personnel information system has undergone training and needs to be implemented and put into operation as soon as possible. 2. Timely report relevant projects regarding policy incentives from superiors. 3. The company should ensure the supply of materials, and all units and departments should strengthen communication. 4. Strengthen coal quality management, produce more and better coal. 5. The salary distribution reflected by each mine should be adjusted according to the unreasonable aspects this year. Measures should be taken to report to the company before November 20th, and after the company's research, it will be implemented in January next year. 6. Next year's mining and production plans should be realistic and minimize deviations as much as possible. 7. The punishment for work-related injuries in each mine should be reasonable. 8. The personnel in each department of the company are basically fully equipped, and the work should be in place. They should do their own work well according to their job responsibilities, and work together to seek common ground while retaining differences.

8、 Li Qunduo, Secretary of the Party Committee and Chairman of the Trade Union, proposed: 1. Seriously strengthen the study of the new "Work Safety Law" and implement various work in accordance with the requirements of the new "Work Safety Law". 2. Recently, the COVID-19 epidemic has occurred in many parts of the country. All units should strengthen publicity and personnel management, take relevant measures to do a good job in epidemic prevention and control, and ensure that the whole company is not affected by the epidemic. Do not travel outside. If there are special circumstances that require going out, you must report according to regulations. When returning, you must strictly follow the procedures of epidemic prevention and control measures. Only when there are no problems can you go to work. 3. Safety management should be in place to reduce the occurrence of work-related accidents. When dealing with work-related injuries, it is necessary to first check for any fractures in the body, especially in areas such as the head, chest, internal organs, eyes, and ears. If there are problems or fractures, it is necessary to first report for work-related injury recognition to avoid further trouble and compensation in the future. Arrange rehabilitation work for serious work-related injuries. 4. Each unit should clarify the situation of work-related injury insurance for employees, and cannot choose not to purchase insurance without going to work, especially for those handled by the team, which must be verified clearly, otherwise it will cause trouble. 5. The work injury management measures of each unit should be practical, and punishment is a measure and means, but some units impose too heavy penalties. 6. To do a good job in occupational disease prevention and control, some front-line old employees should be adjusted and replaced according to their actual situation to avoid causing occupational disease hazards to employees. At present, veteran employees working underground for a long time may inevitably lead to pneumoconiosis or silicosis. Therefore, the annual on-the-job physical examination in each coal mine should be arranged according to the actual situation. 7. Strict management, especially on-site management, must be in place to reduce the occurrence of work-related accidents and effectively safeguard the right to life and health of employees. Most of the work-related injuries that occurred last month were caused by inadequate work management.   Each unit should have a proactive and forward-looking spirit, take initiative and plan ahead, and implement various safety management tasks with a nail in the nail spirit. They must be diligent and fulfill their responsibilities seriously, and achieve the goal of "doing one thing with one heart and one mind" in the development of the enterprise. 9. All work should be closely coordinated, ideologically unified, mutually coordinated, and effectively carried out to promote the rapid development of the company. Do not engage in buck passing or mutual shirking. The members of the team should work together in a cohesive manner to effectively promote the overall work.

9、 General Manager Li Wen put forward the following requirements: 1. According to job responsibilities, ensure the implementation of various tasks and effectively carry out safety work. Currently, there is an upward trend in work-related injuries, and all units should carefully analyze the existing problems and do a good job in the next step of safety prevention. Safety work must be strict, meticulous, practical, and in-depth. Problems must be promptly identified and resolved, and the implementation of regulations must be meticulous. 2. Management personnel should perform their duties according to their job responsibilities, while also fulfilling their responsibilities and ensuring the implementation of the responsibilities of personnel at all levels within their respective areas of responsibility. 3. Strengthen on-site management, implement responsibilities, enhance communication and coordination, and do a good job in source prevention and control. 4. Strengthen the prevention and control of water disasters, and do a good job in prevention work. 5. Carry out special inspections on the "one communication and three prevention" measures, and focus on drilling and drainage work. 6. Strengthen the management of mechanical and electrical transportation to provide transportation support for safe production. 7. On the premise of ensuring safety, each mine carefully organizes production, produces more coal, produces better coal, and increases the company's economic benefits. 8. Conduct a thorough review of the safety production license for Honghui Coal Mine. 9. Organize the company's technical personnel to actively participate in the "Panjiang Cup" Coal Mine Youth Engineering and Technical Personnel Safety Regulations and Measures Competition in Liupanshui City, and reward those who win the competition.

10、 Gao Yongxiong, Vice Chairman and Director of the Supervision Committee, put forward the following requirements: 1. Strengthen the prevention and control of the COVID-19. According to the requirements of epidemic prevention, all units and departments should strengthen the management of personnel, reduce the gathering of personnel, stay away from other places except under special circumstances, and improve the vigilance of epidemic prevention. 2. Throughout the company, the general manager should be at the center, and various management tasks should be carried out effectively. It is strictly prohibited for everyone to act independently and there should be no situation where orders are not followed or prohibitions continue. 3. The acceptance data of Mangyuan Coal Mine should be contacted with the design institute as soon as possible, and the data should be unified without any errors. 4. The company's supervisory committee has recently dealt with a violation incident and dealt with relevant personnel. All units should attach great importance to it, organize learning seriously, strengthen education for management personnel at all levels, and strictly prohibit similar situations from happening. Please make public the phone numbers of the members of the supervisory committee in each mine, and welcome all employees to supervise and report. 5. Each unit should attach importance to the work of the company's supervisory committee and management, cooperate with the work, and do a good job of work coordination.

11、 Chairman Fan Pengfei proposed the following requirements: 1. More efforts should be made in safety management. Currently, there are many work-related injuries in Zhongheng Coal Mine. If work-related injuries occur frequently and cannot be controlled, major problems will arise. Zhongheng Coal Mine should seriously study and reduce the occurrence of work-related accidents. 2. In terms of production, although the raw coal has completed the plan, the coal quality is poor and there is a shortage of clean coal. While increasing production, it is even more important to ensure coal quality. Managers at all levels should fulfill their job responsibilities and seize the current good market opportunities. 3. The key to the development of a company lies in the joint efforts of everyone. The company does not nurture lazy or idle people. The discipline of the entire company must be strict, and management must be controlled and implemented layer by layer, with strict management within the scope of responsibility. 4. All departments and units of the company must carefully prepare the materials for attending the meeting, summarize and arrange their work, and ensure the quality of the meeting. 5. The unreasonable parts of this year's salary distribution plan should be adjusted in a timely manner to facilitate implementation in January next year. 6. In terms of cost control, feasible methods and plans should be proposed, and the company and personnel from various mining operation offices should go deep into the site to understand the actual situation more. Present next year's business management implementation plan by the end of November.
